I would suggest you to read this:
http://wiki.squid-cache.org/ConfigExamples/Authenticate/Ntlm

not just because you need to read but since there are options you don't
use which makes it a bit weird.
like:
auth_param ntlm use_ntlm_negotiate on

It should work for you.
