[squid-users] blocking ads/sites not working anymore?

Hi everyone

Over the time I have collected a lot of sites to block. ads/malware/porn etc. This has been working like a charm. I have even created a
custom errorpage for this.
But since I don't know when, this has stopped working. And according to the googling I have done, my syntax in squid.conf are correct.
So what can be wrong here?

This is my setup:
