My bug. I did not realize freeAllContexts() frees the request. The fix,
similar to what you have described, has been committed to the
DynamicSslCert branch (r9342).

Your debugging and analysis made the fix trivial. Do you mind if I add
you to the Squid CONTRIBUTORS file?
