Should be trivial to add this header in the ICAP Request sent by Squid.
I haven't looked at the code for this in Squid-3 but can't imagine it's
more than one or two lines... (maybe 4 if you want it configurable)

Should also be fairly easy for Trend Micro to add a IWSS config option
making IWSS not require this non-standard ICAP header. Perhaps there
even is one somewhere...
