Try something like:

/usr/lib/squid/ldap_auth
   -R
   -b "dc=my,dc=domain"
   -D "cn=Administrator,cn=Users,dc=my,dc=domain"
   -w "Administrator's_Password"
   -f sAMAccountName=%s
   -h IPofLDAPServer
