But how it would be left the sentence? in the case of user and password I
use this: (squid.conf)

auth_param basic program /etc/squid/sqauth.sh
authenticate_ttl 15 minutes

(and the sqauth.sh makes the control of user and password on mysql)

IP is possible via the exernal acl type. You just need to make your own
helper querying your mysql table..

MAC addresses is not supported in external acls at the moment. Shouldn't
be too hard to add if needed.
