Patches normally have instructions on the sources you can apply it on
and also how to apply it.

mkdir buildarea
cd buildarea
get squid source
get the patch
tar yxvf squid-2.5.STABLE13.tar.bz2
cd squid-2.5.STABLE13
patch -p1 < ../patchfile # the value after the p is specified on the
                         # instructions on how to apply the patch.

./configure --with-your-options
make install

-Wash
