That is not really related to the question since it's apparently not
CONNECT requests which get denied. And the OP's http_access lines look okay
to me since only those CONNECTs which go to ports other than 443 and 563
are denied. (CONNECTs can otherwise easily be abused to tunnel connections
that carry far more than HTTP.) This is also close to the default
configuration.
