That's a browser issue, not a Squid issue. Currently the only
setup that can work that way is IE with NTLM authentication
against a Windows/Samba domain.

There was a long discussion recently about other ways to
implement this without using NTLM auth (which is horribly broken).

You can use the -f option to have the ldap helper search. Check the
archives - this has come up before in the last couple of months.
