This is probably a long-time bug in Squid.
There is currently a open bugzilla entry for this problem.

As of yet no definite fix have been found to the problem.
It would be great if you could try to get a stack trace of your system
to see if it resembles the trace shown in the bugzilla entry.
What cache_dir type are you using? It is a long shot but if you are
using aufs then it might maybe caused by the aufs corruption bug
recently found.. see http://www.squid-cache.org/Versions/v2/2.5/bugs/
for a list of current Squid-2.5 patches.
Any others seeing this problem with Squid-2.5?
