NCAS style password files are creating using the "htpasswd" tool.

Note: if you are using the htpasswd tool from apache then you may need
to use the -d option to tell htpasswd to generate the correct format of
the password file (modern Apache's defaults to MD5, not crypt(), NCAS
style password files uses crypt())
