Have you configured any IPCHAIN rules on your Linux box to redirect all of
the traffic coming on port 80 to the port your Squid is running on?
What is in your squid.conf against following directives:
httpd_accel_host ?
httpd_accel_port ?
httpd_accel_with_proxy ?
httpd_accel_uses_host_header ?
