Squid 2.3 Stable1 problems updating information in cache....

Hi!
 
I have had a problem that I am dying to know why happened. But first my
config, I got a Slackware 7.0 server as a firewall (ipchains) with a
redirect to proxy (transparent) for my internal clients. The rest of the
protocols are managed by ipmasq.
 
The problem is a web site that we use frequently that changed the
default.htm file. The change was that it was doing a redirect to another
document called default_start.htm and now the new file redirects you to
default_exp.htm. When this change was done nothing worked cause in the cache
the redirect called a missing document.
 
Why didn't the server update this information? I let it be for about 2
weeks, and it still didn't update, as a last resort I had to purge the htm
document from the cache.
 
Could someone please advise me on what to do! ?? ! I don't want this to
happen again....
