The cache index file in Squid 2 is located in each cache_dir

  cache_dir/swap.state

You can move the cache index files out of the cache directories by using
cache_swap_log. The name you specify to cache_swap_log will be suffixed
by .<cache_dir_numer>.

The squid.conf comment on cache_swap_log is out to date. Squid 2 uses
separate cache indexes for each cache_dir.
