check your store_avg_object_size. If your actual average size falls
below way below the configured walue then Squid fails in this way (it
can also happen if a small cache gets a lot of requests very fast).
Btw. This should be changed to a non-fatal error causing Squid to take a
temporary pause while it frees some files..
