Check your access.log for repeated queries to sites that are down or
have other trouble (ERR_*). The warning could mean you have connectivity
problems; it could also be caused by a single unreachable site if the
cache isn't very busy.

Yikes -- never seen one that high :-) Are there numerous sites
with ERR_* in access.log, or just a few?
