Improved and updated Squid timer patch

7 months after the previous release I've finally updated the Squid timer
patch, see

It's relative to 1.1.10 which I'm still using for production work.

New:

- Code is now #ifdef'd;
- Individual counters can be switched on and off in timer.h;
- Reports only non-zero counters each report interval;
- Now also measures the open(2) call;
- timerstat.pl script to summarize the results.
