ERR_READ_ERROR on squid 1.1.8

Hi All,

Sorry for my bad English .. :-]
I have squid V1.1.8 on Linux 2.0.0 with 64MB ram, 132 MB swap disk and 24GB of Cache Swap.

I have often on error when a first user's request is send to the proxy :
"ERR_READ_ERROR"
and when the user reload the page the request is good !!
With cachemgr, in cache information, i have the information :
"Page faults with physical i/o: 47862"

Could you help me please !!
